The State of New York requires that each patient must see a physician, nurse practictioner, dentist or podiatrist and receive a prescription for physical therapy. That prescription must be presented to the therapist at your first physical therapy session.

After obtaining your prescription for physical therapy, call one of our offices and we will set up your appointment. When calling, please be prepared to provide the following information:

• Name, date of birth, phone number
• Injury
• Date of injury
• Date of prescription
• Name of MD
• Referral number (if appropriate)
• Type of insurance
• Employer, if using worker's compensation
