Scheduling
New patients are scheduled promptly and can usually be evaluated within 24-48 hours of initial contact.
Referrals
PT Plus welcomes patients from many physician specialties including family practice, orthopedics, neurosurgery, neurology, general surgery, hand surgery and podiatry.
PT Plus abides strictly by New York State law and requires a signed and dated referral from the doctor in order to treat our patients.
Per New York State law, all Medicare patients need to be seen at least every 30 days by the referring MD in order to continue with therapy. Workers comp patients need a follow-up visit every 45 days in order to continue with therapy.
Referrals can be made with specific requests for type of treatment, frequency or duration - or can be written as, "Evaluate & Treat," based upon the professional discretion of the treating therapist.
An initial evaluation report is completed and dictated by the therapist and sent to the referring physician shortly after the first appointment. Periodic progress notes are sent or faxed to the MD prior to the patient's follow-up visits with the physician.
